SHA Care Services recognises that having a chronic disease, or life-limiting illness can take away a lot of freedoms. SHA Care Services think it's important that each individual will always have a choice over how they would like to receive comfort and care to help manage their quality of life. Our palliative care support is in 5 stages, with each stage offering a range of medical, emotional, spiritual and social support services:
Stage 1: Stable
– Developing and Implementing the Care Plan. Clearly, we would like to start our care as soon as possible but recognise this is not possible sometimes.
Stage 2: Unstable
– Adjusting the Care Plan & preparing for the emotions of the future.
Stage 3: Deteriorating
– Shifting to End-of-Life-Care where we aim to make daily life as comfortable as possible.
Stage 4: Terminal
– Symptom Management, Emotional & Spiritual Care if required.
Stage 5: Bereavement
– Support for Family Members, Loved Ones & Carers.
Ultimately, SHA Care Services recognises that it cannot change the outcome but we can affect the journey for each patient, which we will do to the full benefit of the individual.
